At EvolveWell, our mission is to help people maximize the quality of their daily life by making sustainable changes to their environment, their abilities and their routines. We saw a need in our own communities to address issues of health disparity. With more and more people living with chronic health conditions, neighbors, friends and loved ones are experiencing barriers to participate fully in life and live comfortably in their homes.

Why occupational therapy?

Occupational therapy provides a unique solution by taking an individualized whole-person approach. We assess your environment, your physical and mental abilities along with the emotional and motivational components that affect your ability to do the things you want, need and are expected to do. At EvolveWell, we want you to live your best life!
